Booking your Appointment

You will receive a confirmation email when you have completed your form. Directions are sent out with this form, please read.
If you have had your claim registered elsewhere please bring it in, so you are not charged an administration fee.

If you would like a call-out appointment or an after-hours appointment feel free to either call or email and we can work out a time to alleviate your issue.
If your ACC number is older than one year, there will be an additional administration fee charged to you to do an ACC32.
